All contractors must have the following on file in the parish office:

  • completed and signed IRS W-9 form

  • SCS Vendor Information Profile

    • click here to register as a vendor/independent contractor

  • Virtus Certificate (if work is related to ministry)

    • If you still need to become virtus certified, please follow the link to find a class

      • virtus website

    • If you’re certificate is still active please upload a copy here or submit directly to the parish office.
